I understand that submitting this application does not automatically register me as a Grand Valley Yoga Festival volunteer. By submitting this form, I attest that the information I have provided on this application is true and accurate.
If I am accepted as a volunteer, I agree to the following:
To satisfactorily complete all of my scheduled shifts in their entirety and to arrive on time to my scheduled shifts.
To NOT be under the influence of alcohol or drugs during my shifts.
To NOT bring any pets with me to my shifts or volunteer meetings.
To carry my manager's phone number and my schedule with me at all times.
I understand if I fail to meet any of the above requirements it may result in the forfeiture of my pass and/or deposit.
I understand that camping and lodging are not provided to volunteers.

THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR LEGAL RIGHTS!
Release and Waiver of Liability: This Release and Waiver of Liability (the Release ) in favor of Grand Valley Yoga Festival its directors, officers, employees, volunteers, contractors, agents, insurers, lessors, lessees, and any of Grand Valley Yoga Festival or their successors and assigns (collectively, Grand Valley Yoga Festival). The Volunteer desires to work as a volunteer for Grand Valley Yoga Festival and engage in the activities related to being a volunteer (the Activities). The Volunteer understands that the Activities may include but are not limited to constructing and breaking down yoga space, working with or near lighting or electrical equipment, working in the Grand Valley Yoga Festival registration and retail spaces, working in and around public streets, working in studios, gyms or tented space, and lifting, loading and unloading heavy objects. The Volunteer hereby freely, voluntarily, and without duress executes this Release under the following terms and conditions:
Release and Waiver: Volunteer does hereby waive, release and forever discharge and hold harmless Grand Valley Yoga Festival from any and all liability, damages, claims, causes of action and demands of whatever kind or nature, either in law or in equity, which may arise or result directly or indirectly from Volunteer's Activities with Grand Valley Yoga Festival. Volunteer understands that this Release discharges Grand Valley Yoga Festival from any liability or claim that the Volunteer may have against Grand Valley Yoga Festival with respect to any bodily injury, personal injury, illness, death, or property damage that may result from Volunteer's Activities with Grand Valley Yoga Festival, whether caused by the NEGLIGENCE or BREACH OF CONTRACT of Grand Valley Yoga Festival or otherwise. Volunteer also understands that Grand Valley Yoga Festival does not assume any responsibility for or obligation to provide Volunteer with financial assistance or other assistance, including but not limited to medical, health, or disability insurance in the event of bodily injury or illness.
Medical Treatment: Volunteer does hereby waives, releases and forever discharges Grand Valley Yoga Festival from any claim whatsoever which arises or may hereafter arise out of or result directly or indirectly from any first aid, emergency treatment, medical or other service rendered or not rendered to Volunteer in connection with the Volunteer's Activities.
Assumption of Risk: The Volunteer understands that the Activities may include work and occur in locations that may be hazardous to the Volunteer and to others. Volunteer hereby expressly and specifically assumes all risk of bodily injury, personal injury, illness, death or property damage that may arise out of or result directly or indirectly from
Volunteer's participation in the Activities: Volunteer hereby accepts the full legal and financial responsibility for any and all bodily injury, personal injury, illness, death or property damage of any kind which Volunteer may sustain or cause, whether such bodily injury, personal injury, illness, death or property damage results from the NEGLIGENCE or BREACH OF CONTRACT in whole or in part, of Grand Valley Yoga Festival or from some other cause. Volunteer further covenants and agrees NOT TO SUE Grand Valley Yoga Festival on account of any bodily injury, personal injury, illness, death or property damage to Volunteer or any other person which may result directly or indirectly from Volunteer's participation in the Activities.
Insurance: The Volunteer understands that, except as otherwise agreed to by Grand Valley Yoga Festival in writing, Grand Valley Yoga Festival does not carry or maintain health, medical, or disability insurance coverage for any Volunteer. Each Volunteer is expected and encouraged to obtain his or her own medical or heath insurance coverage.
Photographic Release: Volunteer does hereby grant and convey unto Grand Valley Yoga Festival right, title, and interest in any and all photographic images and video or audio recordings made by Grand Valley Yoga Festival during the Volunteer's Activities with Grand Valley Yoga Festival, including, but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings.
Other: Volunteer has carefully read this Release and fully understands and voluntarily accepts the terms and conditions stated herein on behalf of Volunteer and Volunteer's heirs, executors, personal representatives, successors and assigns. Volunteer expressly agrees that this Release is intended to be as broad and inclusive as permitted by the laws of the State of Colorado, and that this Release shall be governed by and interpreted in accordance with the laws of the State of Colorado. In the event of any action or proceeding commenced with respect to this Release, such action or proceeding shall be commenced in the District Court of the State of Colorado, and venue shall be restricted to Mesa County. Volunteer agrees that in the event that any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to be enforceable.
